List of required documents for wholesale drug license:
- Application to DG, DGDA (own company letter head pad) – Detail purpose of this wholesale drug license
- Original copy, Treasury Challan/Govt. fees
- Form-7 (To be collected from DGDA)
- Trade License (Photocopy), YOUR COMPANY NAME. (Drug License will reflect the address mentioned in the Trade License)
- Rent/purchase document of the space (Head office, address can be used) or Recent Electricity Bill copy in favor of YOUR COMPANY. – 300 Taka Non-Judicial stamp
- Bank Solvency Certificate
- IRC (Import registration certificate) (photocopy)
- Tax Return submission proof & TIN Certificate
- National ID Card of Managing Director/proprietor.
- Photographs of the premises.
- Photo of MD/Proprietor – 2 copies
- Lay out plan of the office/warehouse
- Product registration certificate copy. (at least one)
- For Local drug/medical device manufacturer, Drug Manufacturing License Copy
Govt. fees for wholesale drug license in Bangladesh is BDT 11,500.00
Govt. fees for warehouse drug license renewal is BDT 5750.00
Validity of Wholesale drug license: 2 years.
Retail pharmacy/Retail Drug license in Bangladesh:
- Trade License
- Bank Solvency Certificate
- Photocopy of the shop rental agreement, or, Deed of ownership
- NID proof of the owner
- A legalized contract between Pharmacist in-charge & the owner
- TIN Certificate copy of the owner
- Tax Return submission copy
- Form-7 (signed by the pharmacist)
- Registration certificate copy of the Pharmacist
- Photo of the owner
- Photo of the shop
- Govt. fees:
New Allopathic pharmacy: City Corporation & Metropolitan area: 2500 taka + 15% VAT
- New Allopathic pharmacy: Union 1500 taka + 15% VAT
- Renewal of Allopathic pharmacy: City Corporation & Metropolitan area: 1800 taka + 15% VAT
- Renewal of Allopathic pharmacy: Union 700 taka + 15% VAT
- Timeline to get the new retail drug licence: 2-4 months
- Timeline to get the Renewal of retail drug licence: 20 Days
